What's Required...

Because of the affordable pricing on our CPAP and BIPAP machines, many of our customers are medical supply companies, pharmacies, and clinics. However, if you are an individual looking to purchase one of our CPAP or BIPAP machines, your prescription, letter of medical necessity, etc is required before your order can ship.

After your machine order is placed, an email will be sent to you with fax and/or email information that can be used to forward a copy of your prescription, letter of medical necessity, equipment order, etc. 

Our fax number is 507-412-8249 and our email address for this documentation is scripts@secondwindcpap.com. If you do not have a prescription, our physician authorization form can be completed, and returned to us. 

We believe that the physician authorization form is an effective means to 'bridge the gap' for CPAP/BIPAP users who have lost track of their prescription.
